Licensed Practical Nurse for Client Assessments

As a Licensed Practical Nurse (LPN), you will be responsible for conducting comprehensive evaluations and follow-up assessments for clients enrolled in programs for the elderly and disabled.
Core Responsibilities
- Perform initial and annual evaluations in clients' residences, assessing their capacity to perform Instrumental Activities of Daily Living (IADLs) and Activities of Daily Living (ADLs).
- Conduct modified assessments as necessary for transitions from hospitals or nursing facilities, ensuring continuity of care.
- Ensure clients receive the necessary services to remain in their communities, adhering to established guidelines.
- Maintain regular communication with the administrative team to stay updated on re-assessment schedules and annual reviews.
- Review client medications for suitability, documenting any changes in prescriptions or dosages.
- Record assessment findings in the designated electronic health record system, complying with agency protocols and regulatory standards.
- Engage in weekly team meetings with the Medical Director to discuss new admissions, re-evaluations, and significant health changes.
- Participate in mandatory training sessions to enhance professional development.
- Adhere to corporate compliance standards and report any job-related hazards.
- Follow established safety protocols, including infection control measures and emergency procedures.
- Must possess a nursing degree from an accredited institution or have completed an approved LPN program.
- A minimum of two years of practical nursing experience is required, particularly with elderly and disabled populations.
- Current and valid LPN licensure in the state of practice is mandatory.
- Three years of practical nursing experience is preferred, with a background in governmental waiver programs being advantageous.
