Job Summary

PruittHealth is seeking a skilled Licensed Practical Nurse (LPN) to join our team as an Assessment Nurse. In this role, you will conduct initial and reassessments for members/clients in the elderly and disabled waiver program. Your primary responsibility will be to ensure that members have the necessary services to remain in their community and meet their needs based on guidelines.

Key Responsibilities
  • Assessment and Reassessment: Conduct initial and annual assessments in client/member's home to determine their ability to complete IADLs and ADLs.
  • Service Coordination: Assure members have services to remain in their community and fulfill the fiduciary obligation to meet members' needs based on guidelines.
  • Communication and Documentation: Communicate monthly with administrators to receive reassessment schedules and document assessment entries into the Harmony system within established guidelines.
  • Medication Review: Review medications for appropriateness and make note of any new medications, dosages, and discontinued medications.
  • Team Collaboration: Participate in team meetings with the Medical Director to present approved new admits, reassessments, hospitalizations, and discharges.
Requirements
  • Education: Nursing degree from an accredited college or university, or a graduate of an accredited/approved LPN program.
  • Experience: Minimum of two (2) years practical nursing experience, with experience working with elders and disabled populations required.
  • Licensure: Current and unrestricted LPN licensure in the state of practice.
Preferred Qualifications
  • Experience: Three (3) years practical nursing experience, with previous governmental waiver program experience preferred.
