Position Overview

As a vital member of our healthcare team, you will be responsible for conducting comprehensive evaluations and follow-up assessments for clients enrolled in our elderly and disabled waiver program.

Core Responsibilities

  • Perform initial and annual evaluations in the client’s residence, assessing their capacity to manage Instrumental Activities of Daily Living (IADLs) and Activities of Daily Living (ADLs).
  • Execute modified assessments as required during transitions, including hospital and nursing home discharges, as well as for additional service requests.
Ensure that clients receive the necessary services to remain in their homes while adhering to fiduciary responsibilities to meet their needs in accordance with established guidelines.

  • Maintain monthly communication with the administrative team to obtain the reassessment schedule for the SOURCE and CCSP annual reviews.
  • Review and evaluate medications for suitability, documenting any changes in prescriptions, dosages, or discontinuations.
  • Record assessment findings in the Harmony system, following the protocols set by the agency and regulatory bodies.
  • Request medical and hospital documentation as necessary to aid in determining client eligibility.
  • Participate in weekly team meetings with the Medical Director to discuss new admissions, reassessments, hospitalizations, and any concerns regarding client health or functional status.
  • Engage in mandatory training sessions and adhere to the corporate compliance program.
  • Report any job-related tasks that may involve occupational hazards, including exposure to bodily fluids.
  • Adhere to established safety protocols, including fire safety, smoking regulations, and infection control measures.
Skills and Qualifications

  • Proficient in computer usage with the ability to learn new software applications.
  • Ability to meet deadlines set by the Department of Community Health (DCH).
  • Effective communication skills, including the ability to work with translators for non-English-speaking clients.
  • Maintain open communication with supervisors regarding daily tasks and challenges.
  • Ability to foster positive relationships with colleagues, management, and the public.
  • Commitment to the company’s mission and values, ensuring compliance with all policies and regulations.
  • Report any compliance violations to the appropriate supervisory personnel.
  • Demonstrate consistent attendance and punctuality.
  • Maintain confidentiality of client and company information, adhering to the company’s Code of Conduct.
  • Assist in implementing waiver program guidelines and client objectives in collaboration with the Medical Director and Home First team.
  • Conduct home visits to provide care and services in line with waiver program regulations.
  • Ability to perform job responsibilities effectively, with or without reasonable accommodations, without posing risks to health and safety.
  • Adapt to changes and manage additional projects as needed, demonstrating flexibility to meet the needs of clients.
  • Evaluate the health status of assigned clients in accordance with agency policies and scope of practice.
  • Meet productivity standards and utilize resources efficiently.
  • Carry out additional duties as assigned by the Administrator.
  • Participate in the agency’s in-service education program.

Educational Requirements

A nursing degree from an accredited institution or graduation from an accredited/approved Licensed Practical Nurse (LPN) program.

Experience Requirements

A minimum of two (2) years of practical nursing experience is required, with experience in elder and disabled care preferred.

Licensure Requirements

Current and unrestricted Licensed Practical Nurse (LPN) license in the state of practice.

Preferred Qualifications

Three (3) years of practical nursing experience is preferred, along with experience in governmental waiver programs or equivalent training that provides the necessary knowledge, skills, and abilities.
