Job Summary

PruittHealth is seeking a skilled Licensed Practical Nurse (LPN) to join our team as an Assessment Nurse. In this role, you will conduct initial and reassessments for members/clients in the elderly and disabled waiver program. Your primary responsibility will be to ensure that members have the necessary services to remain in their community and meet their needs based on guidelines.

Key Responsibilities
  • Conduct initial and annual assessments in client/member's home to determine their ability to complete IADLs and ADLs.
  • Complete modified assessments as needed for transfers, hospital, and nursing home discharges, and increased or added services.
  • Assure members have services to remain in their community and fulfill the fiduciary obligation to meet members' needs based on guidelines.
  • Communicate monthly with the administrator to receive reassessment schedules for SOURCE and CCSP annual reviews.
  • Review medications for appropriateness and document assessment entries into the Harmony system within established guidelines.
  • Request medical and hospital records as needed to assist with determining eligibility.
  • Participate in team meetings with the Medical Director to present approved new admits, reassessments, hospitalizations, Home Health admits, and discharges.
  • Attend and participate in mandatory in-services and comply with corporate compliance programs.
  • Report job-related functions/tasks that involve occupational hazards, including exposure to blood and body fluids.
  • Follow established safety regulations, including fire protection and prevention, smoking regulations, and infection control and prevention.
Requirements
  • Nursing degree from an accredited college or university, or a graduate of an accredited/approved LPN program.
  • Minimum of two (2) years practical nursing experience, with experience working with elders and disabled populations required.
  • Current and unrestricted LPN licensure in the state of practice.
  • Three (3) years practical nursing experience is preferred, with previous governmental waiver program experience preferred.
