Licensed Practical Nurse for Client Assessments

As a vital member of our healthcare team, the LPN Assessment Nurse is responsible for conducting comprehensive evaluations and reassessments for clients enrolled in our elderly and disabled waiver programs.
Core Responsibilities
- Perform initial and annual assessments in the client's residence, assessing their ability to perform Instrumental Activities of Daily Living (IADLs) and Activities of Daily Living (ADLs).
- Conduct modified assessments as necessary for transitions from hospitals or nursing facilities, ensuring continuity of care.
- Ensure clients receive necessary services to maintain their independence within the community, adhering to established guidelines.
- Maintain regular communication with administrative staff to coordinate reassessment schedules for ongoing program reviews.
- Evaluate medication regimens for suitability, documenting any changes in prescriptions or dosages.
- Accurately record assessment findings in the Harmony system, following agency and regulatory standards.
- Engage in weekly team meetings with the Medical Director to discuss new admissions, reassessments, and any significant health changes.
- Participate in mandatory training sessions to stay updated on best practices and compliance requirements.
- Adhere to safety protocols and report any occupational hazards encountered during duties.
- Follow established safety procedures while performing tasks and operating equipment.
- Proficient in computer applications and capable of learning new software.
- Ability to meet deadlines set by the Department of Community Health.
- Effective communication skills, including the ability to work with interpreters for non-English-speaking clients.
- Maintain open communication with supervisors regarding daily responsibilities and challenges.
- Foster positive relationships with colleagues and management, demonstrating teamwork.

- Comply with all relevant policies, laws, and accreditation standards.
- Report any compliance issues to the appropriate authorities.
- Exhibit punctuality and regular attendance.
- Uphold confidentiality regarding client and company information, adhering to the Code of Conduct.
- Assist in implementing waiver program guidelines and client objectives in collaboration with the Medical Director and care teams.
- Conduct home visits to provide necessary care in accordance with program regulations.
- Capable of fulfilling job responsibilities without posing risks to health and safety.
- Adapt to changes and manage additional tasks as required, demonstrating flexibility in meeting client needs.
- Assess the health status of clients in accordance with agency policies and practice guidelines.
- Meet productivity standards while utilizing resources efficiently.
- Perform additional duties as assigned by the Administrator.
- Engage in ongoing education and training provided by the agency.
A nursing degree from an accredited institution or completion of an approved Licensed Practical Nurse (LPN) program is required.
Experience Requirements
A minimum of two years of practical nursing experience is essential, with a preference for experience in elder and disabled care.
Licensure Requirements
Must possess a current and unrestricted LPN license in the state of practice.
Preferred Qualifications
Three years of practical nursing experience is preferred, with prior experience in governmental waiver programs being advantageous.
