Position Overview

As a vital member of our healthcare team, the LPN for Assessment Services is responsible for conducting comprehensive evaluations and follow-up assessments for clients enrolled in programs for the elderly and disabled.

Core Responsibilities

  • Perform initial and annual evaluations in the client’s residence, assessing their capacity to manage Instrumental Activities of Daily Living (IADLs) and Activities of Daily Living (ADLs).
  • Conduct modified assessments as necessary during transitions, including hospital and nursing home discharges, and when additional services are required.
Service Assurance

  • Ensure clients receive the necessary services to remain in their communities, adhering to fiduciary responsibilities to meet their needs according to established guidelines.
  • Maintain regular communication with administrative staff to coordinate re-assessment schedules for ongoing reviews.
Medication Management

  • Review client medications for suitability, documenting any changes in prescriptions, dosages, or discontinuations.
  • Accurately record assessment findings in the designated system, complying with agency and regulatory standards.
Team Collaboration

  • Engage in weekly team meetings with the Medical Director to discuss new admissions, re-evaluations, hospitalizations, and discharge planning.
  • Participate in mandatory training sessions to enhance professional skills.
Compliance and Safety

  • Adhere to corporate compliance protocols and report any job-related hazards, including exposure to biohazards.
  • Follow established safety guidelines, including fire safety and infection control measures.
Qualifications and Skills

  • Proficient in computer applications and capable of learning new software.
  • Ability to meet deadlines set by the Department of Community Health.
  • Effective communication skills, including the ability to work with interpreters for non-English-speaking clients.
  • Maintain confidentiality of client information and uphold the company’s Code of Conduct.
  • Demonstrate flexibility and adaptability in a dynamic work environment.
Educational Requirements

A nursing degree from an accredited institution or completion of an approved Licensed Practical Nurse (LPN) program.

Experience Requirements

A minimum of two years of practical nursing experience, particularly with elderly and disabled populations, is essential.

Licensure Requirements

Current and unrestricted LPN licensure in the state of practice is mandatory.

Preferred Qualifications

Three years of practical nursing experience is preferred, along with prior experience in governmental waiver programs or equivalent training.
