Licensed Practical Nurse for Client Assessments

As an Assessment Nurse, you will be responsible for conducting comprehensive evaluations and follow-up assessments for clients enrolled in the elderly and disabled waiver program.
Core Responsibilities
- Perform initial and annual evaluations in the client's residence, assessing their ability to manage Instrumental Activities of Daily Living (IADLs) and Activities of Daily Living (ADLs).
- Execute modified assessments as required for transitions, including hospital and nursing home discharges, and for clients needing additional services.
- Ensure clients receive necessary services to remain in their communities, adhering to fiduciary responsibilities and established guidelines.
- Maintain regular communication with administrative staff to obtain schedules for re-assessments and annual reviews.
- Review client medications for suitability, documenting any changes in prescriptions, dosages, or discontinuations.
- Record assessment findings in the Harmony system, following agency and regulatory protocols.
- Participate in weekly team meetings with the Medical Director to discuss new admissions, re-assessments, hospitalizations, and other critical client situations.
- Engage in mandatory training sessions and adhere to corporate compliance standards.
- Report any job-related hazards, including exposure to biological materials, and follow safety regulations regarding fire prevention and infection control.
- Adhere to established safety protocols while performing tasks and operating equipment.
- Must possess computer proficiency and the ability to learn new software applications.
- Ability to meet deadlines set by the Department of Community Health (DCH).
- Effective communication skills, including the ability to work with translators for non-English-speaking clients.
- Demonstrated ability to maintain professional relationships with colleagues and management.
- Commitment to the company's mission and adherence to all policies, procedures, and legal requirements.
Must hold a nursing degree from an accredited institution or have completed an accredited LPN program. A minimum of two years of practical nursing experience is required, with a preference for experience working with elderly and disabled populations.
Licensure
Current and unrestricted LPN licensure in the state of practice is mandatory.
Preferred Qualifications
Three years of practical nursing experience is preferred, along with experience in governmental waiver programs or equivalent training that provides the necessary skills and knowledge.
