Position Overview

As a vital member of our healthcare team, you will be responsible for conducting comprehensive evaluations and re-evaluations for clients enrolled in programs designed for the elderly and disabled.

Core Responsibilities

  • Perform initial and annual assessments in clients' residences, evaluating their ability to manage Instrumental Activities of Daily Living (IADLs) and Activities of Daily Living (ADLs).
  • Conduct modified assessments as necessary for transitions from hospitals or nursing facilities and for changes in service requirements.
Client Support

  • Ensure that clients receive the necessary services to maintain their independence within the community, adhering to established guidelines.
  • Maintain regular communication with administrative staff to coordinate re-assessment schedules and annual reviews.
Medication Management

  • Review client medications for suitability, documenting any changes in prescriptions, dosages, or discontinuations.
  • Accurately record assessment findings in the designated electronic health record system, complying with agency and regulatory standards.
Team Collaboration

  • Participate in weekly team meetings with the Medical Director to discuss new admissions, reassessments, hospitalizations, and any health concerns affecting clients.
  • Attend mandatory training sessions to stay updated on best practices and compliance regulations.
Safety and Compliance

  • Adhere to safety protocols, including infection control measures and emergency procedures.
  • Report any workplace hazards or incidents to the appropriate supervisory personnel.
Qualifications

  • Must possess a nursing degree from an accredited institution or be a graduate of an approved LPN program.
  • A minimum of two years of practical nursing experience is required, with a preference for experience in geriatric and disabled care.
  • Current and valid LPN licensure in the state of practice is mandatory.
Preferred Qualifications

  • Three years of practical nursing experience is preferred, particularly within governmental waiver programs.
