Position Overview

ROLE SUMMARY:

The Licensed Practical Nurse (LPN) plays a crucial role in assessing the needs and conditions of residents, collaborating with the care plan coordinator to formulate personalized nursing care strategies for each individual.

KEY RESPONSIBILITIES:

  • Execute administrative tasks and resident care responsibilities as instructed.
  • Regularly evaluate service plans in conjunction with the supervisor and healthcare team members, participating in necessary meetings.
  • Prepare monthly evaluations for each resident.
  • Document and relay physicians' orders to the resident's medication and treatment records, in consultation with the Director of Nursing (DON).
  • Facilitate the admission process for new residents by assessing their needs and reviewing medical directives.
  • Communicate dietary requirements to the nutrition department.
  • Organize and maintain resident charts, ensuring all necessary documentation is included.
  • Request supplies, equipment, linens, and emergency medications to fulfill unit requirements, ensuring all equipment is functional in collaboration with the DON and Administrator.
  • Assess and provide feedback to Certified Nursing Assistants (C.N.A.s) as necessary.
  • Conduct physical assessments on assisted living and independent residents as per physician directives or following incidents.
  • Adhere to departmental policies and procedures, effectively communicating them to staff, residents, medical personnel, and the public.
  • Exchange nursing reports during shift changes and manage narcotic counts at the beginning and end of shifts.
  • Respond to telephone inquiries professionally.
  • Order necessary medications, supplies, and equipment according to established policies.
  • Coordinate and arrange for diagnostic and therapeutic services as prescribed by the resident's physician.
  • Complete required medical chart documentation.
  • Assist in scheduling appointments and organizing transportation for residents to various departments or facilities for medical services.
  • Manage discharge or transfer processes for residents, ensuring all necessary arrangements and documentation are completed in accordance with facility policies.
  • Support the orientation and training of nursing staff as needed.
  • Maintain a professional demeanor consistently within the unit and towards nursing care and staff.
  • Notify the Administrator of any resident transfers, discharges, or passings.
  • Ensure compliance with infection control protocols and universal precautions during all duties.
  • Follow up on new orders, laboratory tests, imaging, or other diagnostic evaluations on a daily basis.
  • Prepare medication boxes for residents and administer medications as per physician orders.
  • Assist with resident transitions between different levels of care within the organization.
  • Document all activities, complete incident reports, and provide detailed shift reports for continuity of care.
  • Perform additional duties as assigned.
