Key Responsibilities:
The Licensed Practical Nurse (LPN) will provide essential patient care within our health and rehabilitation center, assisting residents with daily living activities under the guidance of a registered nurse.

• Implement individualized care plans for each resident.
• Supervise and evaluate the performance of Certified Nursing Assistants (CNAs).
• Manage the Falls Prevention and Pressure Ulcer Prevention Programs.
• Deliver nursing services as required.
• Conduct thorough assessments of each resident's health status upon admission and throughout their treatment journey.
• Perform additional duties as assigned.

Qualifications:
• Must hold a current, unencumbered state license to practice as an LPN.
• Previous experience in skilled nursing or rehabilitation settings is preferred.
• Candidates should be compassionate, qualified, and committed to excellence in care.
