The Advanced Practice Nurse (APRN) plays a crucial role in delivering exceptional and comprehensive patient care within the Chapters Health System. This position is more than just a job; it is a commitment to enhancing the quality of life for patients and their families.
Key Responsibilities:
- Deliver high-quality direct primary or consultative patient care in alignment with established productivity standards across various clinical environments.
- Collaborate closely with physicians from the medical staff, adhering to protocols while reporting on clinical care provided.
- Engage in interdisciplinary team discussions to ensure optimal diagnosis and medical management, maintaining continuity and quality of care.
- Consult with primary and attending physicians to integrate hospice care with other medical services.
- Work alongside the Patient Access Department to facilitate timely evaluations for hospice services.
- Participate in after-hours and weekend patient care as needed.
- Document all administrative activities thoroughly and accurately to support appropriate coding for services rendered.
- Attend medical staff meetings regularly and review minutes from any missed meetings promptly.
- Prescribe medications as necessary to manage symptoms associated with terminal illnesses.
- Lead initiatives to develop, implement, and assess standards of practice within the hospice specialty.
- Perform additional duties as assigned.
Qualifications:
- Master's degree in nursing.
- APRN certification recognized for Medicare billing.
- Licensure to practice as an APRN in the relevant state.
- Current DEA license and Medicare/Medicaid numbers.
- Active BLS certification for healthcare professionals.
- Extensive knowledge and experience in medical practice and symptom management.
- Advanced clinical skills for thorough patient assessments.
- Ability to work collaboratively with multiple physicians in diverse settings.
- Proficient in basic computer skills, including email and word processing.
- Strong interpersonal, communication, and conflict resolution skills.
- Capability to manage multiple priorities and handle emotional stress effectively.
- Ability to work independently while exercising discretion and judgment.
- Willingness to travel to various locations as required.
- Valid driver's license and reliable transportation.
- Physical ability to access homes in various conditions.
Physical Demands:
This role requires the ability to see, hear, talk, walk, and use hands for various tasks. Frequent standing, sitting, reaching, lifting, and bending are necessary, along with occasional climbing and stooping.
