Job Summary

Mederi Private Care is seeking a skilled Pediatric Licensed Practical Nurse to join our team of healthcare professionals. As a Pediatric LPN, you will provide high-quality nursing care to pediatric patients in their homes, working closely with our interdisciplinary team to deliver comprehensive care.

Responsibilities
  • Provide and direct provisions of nursing and personal care to pediatric patients in their homes, as prescribed by the physician or as authorized by the state/program.
  • Coordinate the plan of care with other healthcare professionals involved in care, helping to achieve and maintain continuity of patient care.
  • Develop and revise individualized plans of care and/or service plans according to federal and/or state program requirements, with other community providers.
  • Ensure plan of care frequency and duration meets the needs of the patient, and initiate plan of care revisions as needed with physician approval.
  • Complete clinical nursing assessments in accordance with federal and/or state program requirements and as required by payer.
Requirements
  • A minimum of one year experience as an RN, with pediatric experience preferred.
  • Current RN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification required.
