We are seeking a highly skilled and dedicated Case Manager/Ongoing Service Coordinator to join our team at Heavenly Hands.
Job SummaryThe Case Manager/Ongoing Service Coordinator will provide ongoing support to our participants through monthly telephonic visits and four in-person visits. This role requires strong coordination and communication skills, as well as the ability to work independently and as part of a team.
Key Responsibilities- Coordinate multiple services among multiple providers to ensure seamless delivery of care.
- Secure annual level of care assessments to ensure participants receive the necessary support.
- Ensure team meetings are scheduled and held as designated in the service plan.
- Facilitate the acquisition, oversight, and delivery of services, ensuring annual service plans are completed in a timely manner.
- Facilitate the waiver applicant/participant's Plan of Protective Oversight (PPO) is completed and supports the service plan.
- Conduct monthly visits via telephonic and/or in-person visits with the participant, ensuring regular check-ins and support.
- Maintain accurate records for at least six years after termination of waiver services.
- Respond to participant crises and emergencies in a timely and compassionate manner.
- Address problems in service provision and other matters as requested by the participant or informal support.
- Bachelor's degree and one year of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.
- Associate's degree and two years of experience providing case management/service coordination and information, linkages, and referrals regarding community-based services for individuals with disabilities and/or seniors.
