Catholic Health Service is seeking a skilled Physical Therapist-PD to join our team at St. Charles Hospital. As a Physical Therapist-PD, you will be responsible for evaluating and treating patients with various conditions, including orthopedic and neurological disorders.
Job Summary:The Physical Therapist-PD will work in an outpatient setting, providing individualized care to patients with a range of diagnoses. You will be responsible for assessing patients' needs, developing treatment plans, and implementing interventions to improve their functional abilities.
Responsibilities:- Evaluate patients' physical abilities and develop treatment plans to improve their functional capabilities.
- Implement evidence-based interventions to address patients' physical limitations and promote independence.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Document patient progress and treatment plans in a timely and accurate manner.
- Collaborate with other healthcare professionals to ensure comprehensive care and optimal patient outcomes.
- Master's or Doctoral degree in Physical Therapy from an accredited program.
- New York State license and registration as a Physical Therapist.
- CPR certification.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
