Position Summary

The Wound Care Certified Advanced Practice Provider will operate under the guidance of the Medical Director, responsible for assessing, directing, and managing healthcare needs within the home environment, either independently or in collaboration with a referring physician.

Key Responsibilities

The ideal candidate will possess outstanding interpersonal abilities to facilitate communication with physicians and interdisciplinary team members. Responsibilities include delivering in-home care to patients with varying levels of acuity and adapting to a new healthcare delivery model aimed at improving access to services. Core Duties Include:

  1. Conducting initial patient visits at home to explain services, assess needs, and coordinate care with the referring physician.
  2. Performing comprehensive assessments, maintaining thorough documentation in electronic medical records (EMR), and executing procedures such as sharp debridement, incision and drainage (I&D), and biopsies as necessary.
  3. Administering wound care, consulting with the ordering physician, and managing orders for related services to enhance wound healing outcomes, including diagnostic tests and specialty referrals.
  4. Interpreting diagnostic results and recommending appropriate follow-up actions.
  5. Providing recommendations for necessary medications, equipment, and supplies to support wound healing and prevention.
  6. Facilitating support groups focused on physical and emotional health topics, and coordinating health promotion activities.
  7. Serving as a resource for decision-making, planning, and policies that impact patient health.
  8. Maintaining accurate patient records and compiling reports regarding home care.
  9. Collaborating with staff to identify patient needs and develop plans to address them.
  10. Following patients during home hospitalizations as required for continuity of care.
  11. Participating in committees related to healthcare initiatives.
  12. Utilizing technology effectively for telehealth consultations.
  13. Performing additional duties as assigned.

Minimum Qualifications

1. Master’s Degree in Nursing or Physician Assistant Studies or a related field is required.

2. A minimum of 3 years of experience as an advanced practice provider is required.

3. A valid Advanced Practice Registered Nurse License or Physician Assistant License in the State of Texas is required.

4. Certification as a Family Nurse Practitioner, Adult Gerontology, or Acute Care Nurse Practitioner, or Physician Assistant Certification from the National Commission on Certification of Physician Assistants is required.

5. Certification in Basic Life Support (BLS) from the American Red Cross or American Heart Association is required.

6. Certification in Advanced Life Support (ALS) from the American Red Cross or American Heart Association is required.

7. Positive references from previous employers and/or professional peers.

8. Successful employment verification.

9. Successful licensing verification.

10. Successful education verification.

11. Successful criminal background check.

12. Successful drug screening.

Working Environment

This position operates within a home healthcare setting. The individual may encounter various environmental conditions, bloodborne pathogens, and physical hazards. Travel by automobile is necessary.

Equipment Utilized

This role regularly employs standard office equipment, including computers, telephones, and medical devices such as blood pressure cuffs and stethoscopes.

Physical Demands

While performing the duties of this role, the employee is frequently required to stand, walk, reach, twist, stoop, and kneel. The role necessitates the use of hands for handling and communication, with specific vision requirements including close and peripheral vision.
