Position Overview

Resilient Healthcare is dedicated to enhancing patient care through a combination of virtual and in-person services, leveraging advanced technology to optimize the experience for both patients and healthcare providers.

Reporting directly to the Medical Director, the Wound Care Certified Advanced Practice Provider will independently assess, plan, and manage healthcare needs within patients' homes, collaborating with referring physicians as necessary.

Key Responsibilities

The successful candidate will possess outstanding interpersonal skills to facilitate effective communication with physicians and interdisciplinary team members. This role involves delivering in-home care to patients with varying levels of acuity and adapting to a modern healthcare delivery model aimed at improving access to services.   • Conduct initial home visits to educate patients about available services, assess their needs, and coordinate care with their referring physician.
  • Perform comprehensive assessments, maintain thorough documentation in the electronic medical record (EMR), and execute procedures such as sharp debridement, incision and drainage (I&D), and biopsies as necessary.
  • Administer wound care, consult with the ordering physician, and manage orders for additional services to enhance wound healing outcomes, including diagnostic tests and specialty referrals.
  • Interpret diagnostic results and recommend appropriate follow-up actions.
  • Provide recommendations for necessary medications, equipment, and supplies to support wound healing and prevention.
  • Facilitate support groups addressing topics related to physical and emotional health, and coordinate health promotion activities.
  • Serve as a resource for decision-making, planning, and policy development impacting patient health.
  • Maintain accurate patient records, compile statistics, and conduct research as required by the organization.
  • Collaborate with team members to identify patient needs and develop plans to address them.
  • Provide continuity of care by following patients in home hospital settings as needed.
  • Participate in committees focused on healthcare delivery.
  • Utilize technology effectively for telehealth consultations.
  • Perform other duties as assigned.

Qualifications

  • Master's Degree in Nursing or Physician Assistant Studies or a related field.
  • A minimum of 3 years of experience as an advanced practice provider.
  • Valid Advanced Practice Registered Nurse License or Physician Assistant License in Texas.
  • Certification as a Family Nurse Practitioner, Adult Gerontology, or Acute Care Nurse Practitioner, or Physician Assistant Certification from the National Commission on Certification of Physician Assistants.
  • Certification in Basic Life Support (BLS) and Advanced Life Support (ALS) from recognized organizations.
  • Positive references from previous employers or professional peers.
  • Successful completion of employment, licensing, education, criminal background, and drug screening verifications.
Work Environment

This position operates primarily in patients' homes, which may expose the individual to various environmental conditions, bloodborne pathogens, and physical hazards. Travel by automobile is required.

Equipment Utilized

This role involves the use of standard office equipment such as computers, telephones, and medical devices including blood pressure cuffs and stethoscopes.

Physical Demands

While performing the duties of this job, the employee is regularly required to stand, walk, reach, twist, stoop, and kneel. The role requires the use of hands for handling and communication, as well as specific vision capabilities.
