Position Summary

The Wound Care Certified Advanced Practice Provider will work under the guidance of the Medical Director to assess, plan, and manage healthcare needs in a home environment, either independently or in collaboration with a referring physician.

Key Responsibilities

  1. Conduct initial consultations with new patients in their homes to introduce services, identify healthcare needs, and coordinate care with the referring physician.
  2. Perform comprehensive assessments, maintain detailed documentation in the electronic medical record (EMR), and execute procedures such as sharp debridement, incision and drainage (I&D), and biopsies as necessary.
  3. Administer wound care, consult with the ordering physician, and manage orders for additional services aimed at enhancing wound healing outcomes, including imaging studies, laboratory tests, vascular assessments, specialty referrals, and orthotic devices.
  4. Interpret diagnostic results and recommend appropriate follow-up actions.
  5. Provide recommendations for prescriptions, medical equipment, and supplies necessary for effective wound healing and prevention.
  6. Facilitate support groups addressing topics related to physical and emotional well-being, such as coping with change, loss, and chronic illness.
  7. Serve as a resource for healthcare planning and policy decisions affecting patient health.
  8. Maintain accurate patient records, track statistics, and conduct research as required by the organization.
  9. Collaborate with team members to assess patient needs and develop comprehensive care plans.
  10. Follow patients in a home hospital setting as needed to ensure continuity of care.
  11. Participate in committees focused on healthcare improvement.
  12. Utilize technology effectively for telehealth consultations.
  13. Perform additional duties as assigned.

Qualifications

1. Master’s Degree in Nursing or Physician Assistant Studies or a related field is required.

2. A minimum of 3 years of experience as an advanced practice provider is required.

3. A valid Advanced Practice Registered Nurse License or Physician Assistant License in Texas is required.

4. Certification as a Family Nurse Practitioner, Adult Gerontology, or Acute Care Nurse Practitioner, or Physician Assistant Certification from the National Commission on Certification of Physician Assistants is required.

5. Certification in Basic Life Support (BLS) from the American Red Cross or American Heart Association is required.

6. Certification in Advanced Life Support (ALS) from the American Red Cross or American Heart Association is required.

7. Positive references from previous employers and/or professional colleagues.

8. Successful employment verification.

9. Successful licensing verification.

10. Successful education verification.

11. Clear criminal background check.

12. Clear drug screening.

Work Environment

This position operates in a home healthcare setting. The individual may encounter various environmental conditions, bloodborne pathogens, physical hazards, and chemical hazards. Travel by automobile is necessary.

Equipment Utilized

This role regularly employs standard office equipment such as computers, telephones, and fax machines, as well as medical devices like blood pressure cuffs and stethoscopes.

Physical Demands

While performing the duties of this position, the employee is frequently required to stand, walk, reach, twist, stoop, and kneel. Regular use of hands and fingers for handling and communication is essential. Specific vision abilities required include close vision, peripheral vision, depth perception, and the ability to adjust focus.
