Key Responsibilities

The Case Manager is responsible for delivering high-quality, patient-centered rehabilitative care. This role involves:

  • Providing orientation to patients and their families regarding the case management process, rehabilitation philosophy, and insurance benefits.
  • Coordinating educational efforts for patients and families, ensuring their active participation in care discussions.
  • Implementing individualized treatment plans that align with patient strengths and preferences.
  • Documenting assessments and communicating the needs and preferences of patients and families.
  • Promoting positive patient interactions and ensuring exceptional service delivery.

Qualifications

  • A minimum of 1 year of experience in case management.

Education

  • Associate Degree in Nursing or Nursing Diploma required.
  • Bachelor's Degree in Nursing preferred.

Licenses and Certifications

  • Current Registered Nurse license in the state of practice.
  • Certification in Case Management is preferred.
