Role Responsibilities

The Case Manager (CM) is responsible for ensuring high-quality, patient-focused care by managing rehabilitative services to achieve optimal outcomes. The CM will:

  • Provide orientation to patients and their families regarding the case management process, rehabilitation philosophy, and discharge criteria.
  • Coordinate education for patients and families, encouraging their involvement in discussions about care plans and goals.
  • Implement treatment plans that align with the patient's strengths and preferences, facilitating their active participation in the rehabilitation process.
  • Document findings from discharge planning evaluations and communicate the needs and preferences of patients and families.
  • Promote positive patient interactions that enhance the overall patient experience.

Qualifications

Experience: Minimum of 1 year in case management.

Education: Associate Degree in Nursing or Nursing Diploma; Bachelor's Degree in Nursing preferred.

Licenses and Certifications: Must hold a current Registered Nurse license in the state of practice; Case Management certification preferred.
