Key Responsibilities

In this role, you will:

  • Provide comprehensive orientation to patients and their families, detailing the case manager's responsibilities, rehabilitation philosophy, and discharge criteria.
  • Act as the primary coordinator for patient and family education, facilitating active participation in care discussions.
  • Implement individualized treatment plans that align with patient strengths and preferences, ensuring their involvement throughout the rehabilitation journey.
  • Document assessments and communicate any social, financial, or discharge needs to the relevant parties.
  • Promote consistent, positive interactions with patients to enhance their experience and satisfaction.

Qualifications

Experience: Minimum of 1 year in case management.

Education: Associate Degree in Nursing or equivalent; Bachelor's Degree in Nursing is preferred.

Licenses and Certifications: Must hold a current Registered Nurse license in the applicable state; certification in Case Management is preferred.
