The Role:

As the Vice President of Regional Operations for the Minnesota Market, you will spearhead operational initiatives in our newest market, implementing our Homeward Navigation delivery model. Your focus will be on scaling services efficiently to provide exceptional member experiences and achieving optimal health outcomes.

Key Responsibilities:

  • Oversee all operational aspects of Homeward in the Minnesota market, ensuring effective implementation of our delivery model.
  • Lead a regional team to achieve patient engagement targets and meet financial goals.
  • Develop and refine systems and processes that promote operational excellence and deliver outstanding patient experiences.
  • Establish and nurture relationships with regional partners, including healthcare providers, payers, and community organizations.
  • Collaborate with centralized teams to align on shared objectives and deliver on our mission.
  • Monitor and report on key quality and service metrics to ensure continuous improvement.
  • Engage in regular travel throughout the state to gain insights into the communities we serve and our partners.

Qualifications:

  • Minimum of 5 years in an operational leadership role, demonstrating a commitment to both process and people.
  • At least 8 years of experience in Medicare/Medicaid and risk revenue, with a strong background in primary and ambulatory care settings.
  • Familiarity with healthcare services and payers in the Minnesota region is essential.
  • Exceptional communication, organizational, strategic planning, and leadership skills.
  • A data-driven mindset with a strong focus on experimentation and learning.
  • A resilient attitude and a passion for transforming healthcare.
  • A Bachelor's Degree is required; advanced degrees are a plus.

Preferred Qualifications:

  • Experience in government programs, managed care, or primary care management.
  • Familiarity with quality management methodologies such as Lean or Six Sigma.
  • Background in health plan provider network performance management or population health management.
  • Experience in a startup environment, demonstrating adaptability and clarity in ambiguous situations.
