Position Details

Title: Prevention Case Manager

Reports To: Early Intervention Program Manager

Schedule: Monday to Friday, Flexible Daytime Hours

Compensation: $21.42 - $22.92 per hour

Classification: Hourly, Non-exempt, 40 Hours per Week

The Housing Programs Department is committed to serving households at risk of homelessness in Hennepin County. Our offerings include Prevention, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for families and individuals. By providing a range of services tailored to varying needs, we aim to assist each household in achieving stability and self-sufficiency.

The Prevention Case Manager plays a crucial role in supporting families and individuals facing imminent housing loss and those with a history of homelessness. Responsibilities include evaluating each household's strengths and needs, aiding in the preparation of necessary documentation for RentHelp applications, and overseeing financial management to ensure compliance with program guidelines.

Key Responsibilities:

  • Assist clients in stabilizing their housing situation through intake and assessment processes.
  • Maintain ongoing communication with landlords, negotiate payment plans, and mediate disputes.
  • Provide financial support to families while adhering to program requirements and budget constraints.
  • Assist in program reporting to ensure alignment with contractual obligations.
  • Collaborate with the Early Intervention Program Manager and Director of Housing Programs to meet funding compliance.
  • Build and sustain productive relationships with community partners and service providers.
  • Connect families to essential resources that will remain available after our services conclude.
  • Educate clients on tenant rights and responsibilities, financial literacy, and affordable housing options.
  • Document case notes, data collection, and other required paperwork promptly and accurately.
  • Participate in meetings with funders as necessary.

Additional duties may be assigned as needed.

This position involves approximately 50% direct client interaction and 50% indirect client-related tasks, including documentation, communication with partners, preparation for meetings, and participation in training and community events.

Physical Requirements:

This role primarily involves sedentary work, with occasional walking and the ability to exert up to 10 pounds of force. Candidates must be able to conduct home visits and apartment showings, which may require climbing stairs.

Qualifications:

  • Possession of a valid Driver's License and successful completion of a DHS NetStudy 2.0 background check and driving record check.

  • Strong organizational skills and attention to detail in managing documentation and technology.

Relevant experience may include:

  • Personal experience with homelessness.
  • One year of documented experience working or volunteering with individuals experiencing homelessness.
  • One year of documented experience as a Case Manager supporting families and individuals.
  • A degree in a relevant field such as Social Work, Human Services, or Family Studies.

We encourage applicants with lived experience to apply. However, individuals who have received services from Agate Housing and Services within the last two years are ineligible for employment.

This position requires the use of a personal vehicle for agency-related activities, and candidates must maintain liability insurance coverage of at least $100,000 per person and $300,000 per occurrence. Background checks will be conducted, and a valid Driver's License is required.

Preferred Qualifications:

  • Two years of experience working with families or individuals experiencing homelessness.
  • Lived experience of homelessness.
  • Licensed Social Worker.
  • Fluency in languages spoken by our clients (e.g., Spanish, Amharic, Oromo, Somali, ASL).
