POSITION DESCRIPTION

Title: Supportive Housing Team Lead Case Manager

Reports To: Associate Director of Supportive Housing

Schedule: Monday to Friday, flexible daytime hours, 40 hours per week

Compensation: $24.35 per hour

Classification: Hourly, Non-exempt

The Housing Programs Department assists households at risk of or experiencing homelessness in Hennepin County. Our offerings include Prevention, Diversion, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for families and individuals. By providing programs of varying intensity and duration, we aim to deliver the appropriate level of support for each household to thrive. Our ultimate goal is to eliminate homelessness by helping households stabilize their housing and attain self-sufficiency.

The Supportive Housing Case Manager is primarily tasked with delivering ongoing, structured case management services within home and community settings for participants enrolled in the Supportive Housing program.

KEY RESPONSIBILITIES

  • Deliver home- and community-based case management services to assist participants in securing and maintaining housing, including assessments, goal setting, and regular in-home visits.
  • Oversee multiple funding sources to support participants while maintaining precise records.
  • Collaborate with Housing Locator to aid participants in their housing search and retention.
  • Establish and nurture relationships with landlords, providing support and mediation as needed.
  • Maintain participant documentation and ensure timely and accurate record-keeping, including daily case notes and data collection.
  • Provide consistent and meaningful contact with participants to foster trust, prevent crises, and address individual needs.
  • Conduct monthly home visits for each assigned participant.
  • Develop personalized goal plans with participants and offer necessary support to achieve their objectives.
  • Effectively engage with participants from diverse backgrounds, including youth, individuals with historical trauma, those facing domestic violence, and those dealing with mental, physical, developmental, and substance use challenges.
  • Coordinate services with other program staff and actively participate in team meetings.
  • Offer flexible services that align with participants' needs.

TEAM LEAD SPECIFIC RESPONSIBILITIES

  • Provide guidance, support, and mentorship to team members regarding participant scenarios and daily responsibilities.
  • Facilitate consistent communication with team members about program policies and practices.
  • Collaborate with the Program Manager to enhance program effectiveness and maintain a thorough understanding of client eligibility and funding requirements.
  • Assist in onboarding and training new staff members.
  • Serve as the primary support for case managers.
  • Conduct monthly refresher training on current policies and procedures.
  • Maintain a performance scorecard of 95% or higher.
  • Perform additional duties as assigned.

PHYSICAL REQUIREMENTS

  • Primarily sedentary work, involving sitting most of the time with occasional walking. Must be able to exert up to 10 pounds of force occasionally.
  • Must be capable of attending home visits and property showings, including locations that may require climbing stairs.
  • Must have the ability to drive for home visits and other case-related appointments.

REQUIRED QUALIFICATIONS

  • Valid Driver's License and access to a safe, insured vehicle.
  • Successful completion of a DHS NetStudy 2.0 background check.

Relevant experience that equips candidates with the necessary skills and perspectives may include:

  • Personal experience with homelessness.
  • Work, internship, or volunteer experience with individuals facing homelessness.
  • Degree in a relevant field such as Social Work (licensure preferred), Human Services, or Family Studies.

We encourage individuals with lived experience to apply. However, those who have received services from Agate Housing and Services within the past two years are ineligible for employment.

This position requires the use of a personal vehicle for agency-related activities and maintenance of liability insurance coverage of at least $100,000 per person and $300,000 per occurrence. In addition to criminal background checks, candidates must possess a valid Driver's License and pass a driving record check.

PREFERRED QUALIFICATIONS

  • One year of casework experience.
  • Two years of experience working with individuals experiencing homelessness.
  • Licensed Social Worker.
  • Fluency in languages spoken by our participants (Spanish, Amharic, Oromo, Somali, ASL).
  • Familiarity with community resources relevant to participant needs.

REFERRAL BONUS PROGRAM
