Role Summary:

This role involves collaborating within a team that employs a Harm Reduction and Housing First approach to assist individuals by addressing their unique needs, with a primary focus on securing housing. Team members will provide ongoing support, advocacy, and assistance in accessing community resources to ensure housing stability and community integration. Case Managers must possess the knowledge and skills to effectively navigate the complexities of participants' lives and the co-occurring challenges associated with long-term homelessness.

Our primary focus is on serving BIPOC women, trans, and non-binary individuals who have faced homelessness.

Key Responsibilities:

  • Deliver tailored support to participants in obtaining and maintaining housing, including outreach, networking, and landlord engagement.
  • Provide comprehensive case management, maintaining a caseload of 15-20 participants, offering weekly support, crisis intervention, assessment, and goal-setting.
  • Ensure accurate documentation of all case management activities.
  • Actively contribute as a member of a small, supportive team.

Qualifications:

  • A Bachelor's degree in human services or equivalent experience is required.
  • A minimum of three years of experience working with single adults who have experienced long-term homelessness or personal experience with homelessness.
  • Knowledge of domestic abuse, childhood trauma, grief and loss, sexual violence and exploitation, homelessness, social support systems, advocacy, tenant law, and case management is essential for providing effective services to women in crisis.
  • Experience with mental health and substance use issues is preferred.
  • Proficiency in computer skills is necessary.
  • A valid driver's license and access to a vehicle are required.
  • Home visitation requires awareness of personal safety.

Preferred Qualifications:

  • Experience working with individuals experiencing homelessness or having personally experienced homelessness.
  • A broad understanding of housing resources.
  • Bilingual skills are advantageous.
