Detailed Job Description

Multicultural Care Center LLC is on the lookout for a dedicated Housing Case Manager to join our Housing Stabilization Service (HSS) program. This position focuses on the essential tasks of screening, facilitating housing searches, and coordinating placements, as well as engaging with landlords to assist individuals in securing and sustaining housing within Hennepin and Ramsey Counties.

The successful candidate will demonstrate exceptional skills in building relationships with private landlords and property managers, while also providing comprehensive case management, housing support, and direct assistance. A person-centered, harm reduction, and housing-first methodology is crucial when working with participants from various backgrounds who have experienced homelessness and mental health challenges.

Key Responsibilities:

Conducting housing intake, searches, and placementsCreating tailored housing search and placement strategiesFostering connections with private landlords and property managersLinking households to essential resources and benefitsAssisting with relocation tasks and logisticsDelivering tenancy support services and crisis managementPromoting participant health and well-beingFacilitating income enhancement and transportation access for participantsMaintaining documentation and case records

Qualifications:

  • Bachelor's Degree in Human Services, Social Work, or a related discipline
  • 2-3 years of experience in providing support to individuals facing behavioral health challenges

Salary Range: $21.00-$22.00 per hour, depending on qualifications

Work Environment: In-person

Employment Type: Full-time

Work Schedule: 8-hour shifts
