Position Details
Title: Supportive Housing Team Lead Case Manager
Reports To: Associate Director of Supportive Housing
Schedule: Monday to Friday, Flexible Daytime Hours, 40 Hours/Week
Compensation: $24.35 per hour
Classification: Hourly, Non-exempt

The Housing Programs Department provides support to households facing or at risk of homelessness in Hennepin County. Our services encompass Prevention, Diversion, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for families and individuals. By offering programs of varying intensity and duration, we aim to meet the specific needs of each household, with the ultimate goal of ending homelessness through stabilization and self-sufficiency.
The Supportive Housing Case Manager plays a crucial role in delivering ongoing, structured case management services within the Supportive Housing program.
Key Responsibilities:
- Deliver home- and community-based case management services to assist participants in securing and sustaining housing (including assessments, goal planning, and regular visits).
- Oversee multiple funding sources to support participants while maintaining precise records.
- Collaborate with Housing Locator to aid participants in their housing search and retention.
- Establish relationships with landlords, providing mediation and support in landlord-tenant interactions.
- Maintain participant documentation, ensuring timely and accurate case notes and data collection.
- Engage with participants regularly to foster trust, assist in crisis prevention, and address individual needs.
- Conduct monthly home visits for each assigned participant.
- Develop personalized goal plans with participants, offering necessary support to achieve their objectives.
- Work effectively with a diverse range of participants, including those with unique identities and experiences.
- Coordinate services with other program staff and actively participate in team meetings.
- Provide flexible services tailored to the needs of participants.
Team Lead Responsibilities:
- Offer guidance, support, and mentorship to team members regarding participant scenarios and housing case management fundamentals.
- Facilitate consistent communication among team members concerning program policies and practices.
- Collaborate with the Program Manager to implement program improvements and maintain an understanding of client eligibility and funding requirements.
- Assist in the onboarding and training of new staff members.
- Serve as the primary support for case managers.
- Conduct monthly training sessions on current policies and procedures.
- Maintain a performance scorecard of 95% or higher.
- Perform additional duties as assigned.
Physical Requirements:
- Primarily sedentary work, with occasional walking and the ability to exert up to 10 pounds of force.
- Must be able to conduct home visits and attend apartment showings, including units accessible only by stairs.
- Must possess a valid Driver's License and access to a reliable vehicle for casework appointments.
Qualifications:
- Valid Driver's License and access to an insured vehicle.
- Successful completion of a DHS NetStudy 2.0 background check.

Preferred Qualifications:
- One year of casework experience.
- Two years of experience working with individuals experiencing homelessness.
- Licensed Social Worker.
- Fluency in languages spoken by participants (e.g., Spanish, Amharic, Oromo, Somali, ASL).
- Familiarity with community resources relevant to participant needs.
