Position Description

Title: Family Rapid ReHousing Case Manager

Reports To: Rapid ReHousing Program Manager

Schedule: Monday to Friday, Flexible Daytime Hours

Compensation: $21.42 - $22.92 per hour

Classification: Hourly, Non-exempt, 40 Hours per Week

Union Representation: This role is represented by AFSCME Local 999

The Housing Programs Department at Agate serves families at risk of or experiencing homelessness in Hennepin County. Our services encompass Prevention, Diversion, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for families with children and adults without children. By offering a spectrum of programs, we aim to provide tailored support to each household, with the ultimate goal of ending homelessness through stabilization and self-sufficiency.

Key Responsibilities:

  • Assist families in transitioning from homelessness to stable housing.
  • Conduct housing searches and negotiate with landlords on behalf of clients.
  • Deliver structured case management focused on housing stability through regular home visits after securing housing.
  • Support families in overcoming barriers to housing stability, emphasizing income enhancement.
  • Connect families with ongoing resources beyond our services.
  • Establish and maintain relationships with landlords to facilitate housing access.
  • Mediate any landlord-tenant disputes.
  • Educate clients on tenant rights, responsibilities, financial management, and affordable housing options.
  • Collaborate effectively with agency staff and Hennepin County shelter personnel to address client needs.
  • Manage various funding sources to assist clients while maintaining accurate financial records.
  • Complete timely and accurate case notes, data collection, and documentation for assigned clients.

This position entails approximately 50% direct client interaction and 50% indirect client-related tasks, including documentation, communication with partners, preparation for meetings, and participation in training and community events.

Physical Requirements

This role primarily involves sedentary work, with occasional walking and the ability to exert up to 10 pounds of force. Attendance at home visits and apartment showings is required, including locations that may necessitate climbing stairs.

Qualifications
  • Valid Driver's License and successful completion of a DHS NetStudy 2.0 background check and driving record check.

Relevant experience may include personal experience with homelessness, work, internships, or volunteer roles with homeless populations, or educational qualifications in Social Work, Human Services, or Family Studies.

Agate Housing and Services encourages applicants with lived experience to apply, but individuals who have received services from Agate within the last two years are ineligible for employment.

This position requires the use of a personal vehicle for agency-related activities, with a minimum liability insurance coverage of $100,000 per person and $300,000 per occurrence.

Preferred Qualifications
  • Two years of experience working with individuals experiencing homelessness.
  • Fluency in languages relevant to our client base (e.g., Spanish, Amharic, Oromo, Somali, ASL).
