POSITION DESCRIPTION

Title: Family Rapid ReHousing Case Manager

Reports To: Rapid ReHousing Program Manager

Schedule: Monday to Friday, Flexible Daytime Hours

Compensation: $21.42 - $22.92 per hour

Classification: Hourly, Non-exempt, 40 Hours per Week

The Housing Programs Department at Agate serves families who are experiencing or are at risk of homelessness in Hennepin County. Our services encompass Prevention, Diversion, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for families with children and adults without children. By offering a spectrum of programs tailored to varying needs, we aim to provide the necessary support for each household to thrive. The overarching goal of our housing initiatives is to eliminate homelessness by assisting families in securing stable housing and achieving self-sufficiency.

Key Responsibilities:

  • Assist families experiencing homelessness in Hennepin County in transitioning to stable housing.
  • Conduct housing searches and negotiate with landlords on behalf of participants.
  • Deliver structured case management services focused on housing stability through regular home visits once housing is secured.
  • Support families in overcoming barriers to housing stability, emphasizing income enhancement.
  • Connect families to ongoing resources that will remain available after our services conclude.
  • Establish relationships with landlords to facilitate housing access for clients.
  • Mediate any landlord-tenant disputes.
  • Educate clients on tenant rights and responsibilities, financial management, and affordable housing options.
  • Collaborate effectively with agency staff and Hennepin County shelter personnel to address client needs.
  • Manage various funding sources to assist clients while maintaining accurate expenditure records.
  • Complete timely and accurate case notes, data collection, and documentation for assigned clients.

This position entails approximately 50% direct client interaction and 50% indirect client-related tasks, including documentation, communication with partners, preparation for client meetings, and participation in training and community events.

PHYSICAL REQUIREMENTS:

This role involves sedentary work, primarily sitting, with occasional walking and the ability to exert up to 10 pounds of force. The position requires attending home visits and apartment showings, which may necessitate climbing stairs.

QUALIFICATIONS:

  • Valid Driver's License and successful completion of a DHS NetStudy 2.0 background check and driving record check.

Relevant experience that equips candidates with the necessary skills and perspectives may include:

  • Personal experience with homelessness.
  • Work, internship, or volunteer experience with individuals facing homelessness.
  • Educational background in Social Work, Human Services, or Family Studies.

We encourage applicants with lived experience to apply. However, individuals who have received services from Agate Housing and Services within the last two years are ineligible for employment.

This position requires the use of a personal vehicle for agency-related tasks, along with maintaining liability insurance coverage of at least $100,000 per person and $300,000 per occurrence. In addition to background checks, candidates must possess a valid Driver's License and pass a driving record assessment.

PREFERRED QUALIFICATIONS:

  • Two years of experience working with individuals experiencing homelessness.
  • Proficiency in languages spoken by our clients (Spanish, Amharic, Oromo, Somali, ASL).
