Position Title: Case Manager - Permanent Supportive Housing


Our Saviour's Community Services (OSCS) is dedicated to providing dignified shelter and housing solutions for single adults, alongside essential educational resources for adult English language learners. Founded in the early 1980s, OSCS operates independently as a nonprofit organization, committed to serving the community without religious affiliation.


Our Saviour's Housing manages a 21-bed emergency shelter and a permanent supportive housing initiative for single adults aged 18 and above. The shelter offers essential services, including meals and supportive resources, ensuring a welcoming environment for all genders. Operating continuously throughout the year, our team collaborates to deliver services that foster dignity and hope.


The permanent supportive housing program aims to secure stable housing for 100 single adults or couples, utilizing community resources and onsite facilities. Participants benefit from comprehensive case management focused on housing support and stabilization, with access to transportation and food resources.


Role Responsibilities:

  • Facilitate the housing application and move-in process, ensuring participants have access to necessary resources.
  • Conduct weekly meetings with residents to develop personalized service plans, promoting self-sufficiency and addressing barriers to housing success.
  • Educate residents on landlord expectations and tenant rights, mediating communication and resolving conflicts as needed.
  • Provide crisis intervention to maintain housing stability and promote harm reduction strategies.
  • Assist residents in accessing transportation and essential services, including medical and social support.
  • Guide residents in managing personal finances, living skills, and rental obligations.
  • Onboard residents into Housing Stabilization Services, including the development of Person-Centered Plans.
  • Collaborate with internal and external stakeholders, including county services and outreach organizations.
  • Establish professional relationships with landlords to support residents' housing needs.
  • Assist with intake processes and documentation for housing agreements.
  • Maintain accurate case notes and documentation, adhering to deadlines for reporting and administrative tasks.
  • Participate in regular case management meetings and training sessions to enhance service delivery.
  • Support the training and onboarding of new staff members.
  • Engage in various duties as assigned, adapting to the evolving needs of the organization.
  • Contribute to fostering a culture of belonging within OSCS.


Required Skills and Qualifications:

  • Demonstrated commitment to diversity and cultural fluency.
  • Strong interpersonal skills, with the ability to empathize and connect with individuals.
  • Familiarity with Harm Reduction and Housing First principles.
  • Effective communication skills, both written and verbal.
  • Excellent attention to detail and time management abilities.
  • Valid driver's license and reliable transportation for client visits.
  • Proficient in using technology, including Google Suite and Microsoft Office.
  • Willingness to learn about homelessness and its contributing factors.
  • Physical ability to perform job duties, including lifting up to 50 pounds.
  • Proficiency in one or more languages such as Spanish, Somali, Arabic, Ukrainian, Russian, or Farsi/Persian is preferred.


Experience Requirements:

  • Minimum of 2 years of relevant experience in human services or related fields.
  • Valid driver's license and a good driving record.
  • Ability to pass a criminal background check and maintain compliance with regulatory standards.
  • Familiarity with Homeless Management Information Systems (HMIS) is a plus.
  • Experience in nonprofit organizations or similar service-oriented programs.
