Position Title: Prevention Case Manager
Reports To: Early Intervention Program Manager
Schedule: Monday to Friday, Flexible Daytime Hours
Compensation: $21.42 - $22.92 per hour
Classification: Hourly, Non-exempt, 40 Hours per Week
Union Representation: This role is represented by AFSCME Local 999.
The Housing Programs Department provides support to households at risk of homelessness in Hennepin County. Our offerings include Prevention, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for families and individuals. By providing a range of services tailored to different needs, we aim to help each household achieve stability and self-sufficiency.
The Prevention Case Manager plays a crucial role in assisting families and individuals facing imminent housing loss and those with a history of homelessness. This position involves evaluating each household's strengths and requirements, aiding them in compiling necessary documentation for RentHelp applications, and overseeing expenditures and services to ensure compliance with program standards and budgetary constraints.
Key Responsibilities:
- Support participants in stabilizing their housing situation through intake and assessment processes.
- Maintain ongoing communication with landlords, negotiate payment plans, and mediate landlord-tenant disputes.
- Provide financial assistance to families while adhering to program guidelines and budgetary limits.
- Assist with program reporting to ensure alignment with contractual obligations.
- Collaborate with the Early Intervention Program Manager and Director of Housing Programs to meet funding compliance requirements.
- Develop and sustain active partnerships with community organizations and service providers.
- Connect families to essential resources that will remain available after our services conclude.
- Educate clients on tenant rights and responsibilities, financial management, and affordable housing options.
- Document case notes, data collection, and other necessary paperwork for assigned clients promptly.
- Participate in meetings with funders as required.
Additional duties may be assigned as needed.
This position involves approximately 50% direct client interaction and 50% indirect client-related tasks, including documentation, communication with partners, preparation for client meetings, and participation in training and community events.
Physical Requirements:
This role primarily involves sedentary work, with occasional walking and the ability to exert up to 10 pounds of force. Home visits and apartment showings may be required, including locations accessible only by stairs.
Required Qualifications:
- Possession of a valid Driver's License and successful completion of a DHS NetStudy 2.0 background check and driving record review.

Relevant experience may include:
- Personal experience with homelessness.
- One year of documented experience working, interning, or volunteering with individuals experiencing homelessness.
- One year of documented experience as a Case Manager supporting families and individuals.
- An Associate's, Bachelor's, or Master's degree in a relevant field such as Social Work (with appropriate licensure), Human Services, or Family Studies.
We encourage individuals with lived experience to apply. However, those who have received services from Agate Housing and Services within the last two years are ineligible for employment.
This position requires the use of a personal vehicle for agency-related tasks, and candidates must maintain liability insurance coverage of at least $100,000 per person and $300,000 per occurrence. In addition to criminal background checks, applicants must possess a valid Driver's License and pass a driving record evaluation.
Preferred Qualifications:
- Two years of experience working with families and/or individuals experiencing homelessness.
- Lived experience of homelessness.
- Licensed Social Worker.
- Fluency in languages spoken by our clients (Spanish, Amharic, Oromo, Somali, ASL).
Referral Bonus Program:
If you know someone who would be a great fit for our organization, refer them Agate Housing and Services offers a $250 bonus to employees who refer qualified candidates for current openings. The referring employee must indicate their name in the application section that requests this information.
Referral Bonus Forms must be submitted to Human Resources within 30 days of the referred employee's start date. Forms submitted after this period will not be processed.
To qualify for rewards, candidates must be hired as permanent full-time or part-time employees (this excludes on-call, temporary, intern, or contract positions).
