Position Overview:
Marin Community Clinics, established in 1972, operates as a comprehensive multi-clinic network offering a diverse range of integrated services including primary care, dental, behavioral health, specialty care, and referral services. The Addiction Recovery Case Coordinator (RN) will collaborate closely with primary care providers and other members of the clinical care team, utilizing expertise in addiction and recovery to deliver patient-centered, outcome-focused care that fosters independence and dignity.

Key Responsibilities:
Provide comprehensive case management support and care coordination for a designated group of patients experiencing substance use disorders.
Collaborate regularly with the MAT Clinic Team, behavioral health specialists, and medical providers.

For MAT Clinic:
Initiate Prior Authorizations and TARS for medications to ensure timely access for patients.
Conduct thorough assessments and develop customized care plans for each patient.
Ensure compliance with all program reporting requirements.
Manage and respond to tasks and care needs efficiently.
Facilitate connections and referrals to internal and external resources.
Monitor referrals and provide follow-up to guarantee service delivery.
Assist patients in coordinating all necessary care.
Conduct home and community visits as required.
Collaborate with the interdisciplinary care team, including behavioral health, care navigation, medical assistants, nursing staff, and external partners.
Participate in internal and external meetings, such as case conferences and administrative discussions.
Engage in training sessions covering health coaching, engagement techniques, and more.
Provide timely and accurate reporting of data as requested by MCC CM programs.
Travel between clinics or meet patients in the community as necessary.
Offer thoughtful insights regarding case management services and quality improvement initiatives.
Provide support to satellite clinics as needed.
Additional duties may be assigned.

Qualifications:
Education and Experience: RN, CAC, or equivalent experience.
1-2 years of experience (and a passion for) supporting individuals with substance use disorders is highly preferred.
Familiarity with state regulations and legal/ethical standards related to patient rights and confidentiality is required.
Experience with Electronic Medical Records, particularly EPIC, is desirable but not mandatory.

Required Skills and Abilities:

Must be fluent in English with the capability to read, analyze, and interpret general business publications, professional journals, technical procedures, or governmental regulations.

Proficient in writing reports and business correspondence in English.
Bilingual skills - Must be able to communicate effectively with patients and partners in Spanish.
Ability to present information and respond to inquiries from clients, customers, and the public in Spanish.

Strong preference for candidates who can conduct clinical assessment interviews and case management interventions with patients in Spanish.

Position requires travel to multiple MCC locations in Marin County.

Computer Skills:
To perform this role successfully, an individual must be computer literate and proficient in Microsoft Office software programs, as well as capable of conducting internet searches.

Certificates, Licenses, Registrations:
Current CPR Training
Valid CA Driver's License.

Physical Requirements:
Prolonged standing, walking, and extended periods of sitting at a desk while working on a computer.
Must be able to lift up to 15 pounds when necessary.
Fine motor skills typical of performing standard Addiction Recovery Case Coordinator (RN) duties.
