Your Impact
As a Case Manager, you will play a vital role in transforming the lives of individuals facing substance abuse challenges, often accompanied by co-occurring conditions. This position allows you to develop and apply your skills in compassionate care, crisis intervention, and de-escalation within a supportive and inclusive work atmosphere. You will have the opportunity to provide essential substance abuse treatment, guiding individuals from despair to a place of hope and potential.


Key Responsibilities

  • Deliver case management services to clients at the Detoxification Center.
  • Support Detoxification Department staff in coordinating client withdrawal management needs.
  • Schedule and facilitate client medical, psychiatric, and continuing care appointments.
  • Conduct ASAM screenings and referral assessments as necessary.
  • Maintain accurate documentation in the Electronic Health Record system.
  • Ensure compliance with Case Management billing practices.
  • Provide timely crisis intervention as required.
  • Engage with clients daily to assess case management and referral needs.
  • Participate in consultations with relevant county agencies regarding ASAM and other standards.
  • Foster positive relationships with community providers and managed care organizations.
  • Assist in case management training during monthly staff meetings.
  • Offer clients educational resources and information on continuing care options, including residential treatment and recovery residences.
  • Exhibit sound decision-making skills on a daily basis.
  • Utilize technology for effective communication with peers, supervisors, and community members.
  • Assist with data entry into the EHR system as needed.
  • Maintain a foundational understanding of medications utilized by clients.
  • Drive program vehicles when necessary.
  • Perform additional duties as assigned by the Program and Personnel Manager.

Qualifications

  • Knowledge of alcohol and drug addiction.
  • Strong oral and written communication skills.
  • Proficient computer skills, including data entry into Electronic Health Records.
  • Ability to communicate effectively using electronic systems.
  • Familiarity with substance use programs, hospitals, and other continuing care options.

Experience

  • Experience in crisis intervention with clients in distress.
  • Background in working with individuals with substance use disorders.
  • Extensive case management experience with underserved populations in public settings.
  • Experience engaging with diverse populations and cultures.
  • Experience supporting clients with co-occurring disorders and disabilities.
  • Understanding of customer service principles.
  • Experience in conflict resolution in high-pressure situations.

Additional Requirements

  • Must be registered or certified with a State AOD certifying body such as CCAPP, CADTP, or CAADE.
  • Valid California driver's license with a clean driving record and car insurance.
  • Current CPR/First Aid certification is desirable.
  • Drug testing is mandatory at the time of hire.
  • Compliance with COVID-19 vaccination and booster mandates for healthcare staff.
  • TB Test required.

We offer a comprehensive benefits package for all full-time and part-time employees working over 20 hours per week, including:

  • 100% coverage of medical, dental, life, and LTD for employees, with 50% coverage for dependents.

Salary: $28.00-$30.00/hr based on experience and qualifications.
