Zinnia Health is looking for a full-time Recovery Support Specialist, certified with CCAPP. The Recovery Support Specialist is tasked with delivering direct services and supportive interactions to clients and their families, while also cultivating community resources that facilitate the client's recovery process. This role also serves as an advocate for clients, ensuring they receive appropriate services and assistance in accessing these resources. The Recovery Support Specialist will conduct group therapy sessions and manage related documentation, alongside additional supportive services. This position requires a highly qualified individual with a deep understanding of addiction and recovery processes. Familiarity with 12-Step Recovery programs and proficiency in documenting group notes within an EMR system is essential. The ability to collaborate effectively with team members, including the Primary Counselor and Clinical Director, is crucial for providing high-quality treatment and care.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Coordinate and deliver care that is safe, timely, effective, efficient, equitable, and centered on the patient.
- Manage case assignments, develop service plans, assess case progress, and determine case closure.
- Communicate client needs to various members of the care team and ensure appropriate follow-up.
- Oversee discharge planning upon completion of treatment.
- Participate in clinical and administrative meetings for the exchange of information across departments.
- Collaborate with clients, families, therapists, and referral sources to ensure exceptional care.
- Facilitate various aspects of care coordination and information sharing.
- Maintain comprehensive clinical records for clients through meticulous documentation.
- Assist clients in making informed decisions by advocating for their clinical status and treatment options.
- Promote quality and cost-effective interventions and outcomes.
- Document case information accurately, complete necessary forms, and generate statistical reports.
- Assist clients in navigating legal challenges, including participation in court hearings as needed.
- Adhere to professional standards as outlined by established protocols, rules, and regulations.
- Act as an advocate for clients regarding insurance coverage and financial assistance.
- Continuously monitor and evaluate each client's progress through their case management plan, making necessary adjustments as required.
- Coordinate with social services, health, and governmental agencies.
- Perform additional duties as assigned by the supervisor, consistent with the position and in compliance with agency policies and procedures.
EDUCATION / EXPERIENCE
- CCAPP (or equivalent certification body) certified CADC or higher.
- 1 to 2 years of experience as a group facilitator in Substance Abuse and Mental Health settings.
- Bachelor's Degree in a human services-related field.
- Minimum of 1 year of full-time experience in human services or behavioral health sciences.
- Familiarity with Department of Children and Families (DCF) guidelines and Joint Commission standards.
- Knowledge of 12-Step Recovery programs.
- Experience with Kipu is preferred.
$24-$26 hourly
