Key Responsibilities:

  • Design and articulate comprehensive care management processes throughout the organization.
  • Foster organizational dedication, secure executive support, ensure technology adoption, oversee execution, and monitor essential metrics for the deployment of critical care management strategies and initiatives.
  • Develop an annual budget from a zero-based approach that aligns with the program's care management philosophy and strategic objectives.
  • Build and maintain connections with essential clinical, financial, and ancillary support teams within the organization.
  • Identify key performance indicators and related financial reports to assess care management performance against established plans.
  • Create an effective governance framework for care management across the organization.
  • Maintain and communicate a prioritized project portfolio that aligns with the care management strategy.
  • Lead and support the execution of all initiatives related to care management.
  • Collaborate and establish relationships with clinic leaders, executive teams, and other stakeholders.
  • Enhance provider relationships, cultivate trust, and encourage provider engagement for improved adoption of care management clinical and financial systems related to integrated patient quality management.
  • Provide leadership to execute on system quality, operational, and capital budgets, adhering to agreed timelines and objectives.
  • Achieve or surpass budgeted operating margins.
  • Develop and implement care policies and processes in collaboration with the Chief Medical Officer and Medical Clinic Director.
  • Enhance the competencies of care management staff to ensure proficiency in job-specific competency assessments.
  • Regularly measure and improve provider satisfaction metrics in annual increments.
  • Create and maintain an updated care management policy and procedure manual, including overseeing its implementation.
  • Formulate and execute a comprehensive care management strategy across the clinic.

Qualifications:

  • Bilingual in Spanish is required.
  • Must possess a valid Clinical Social Work license.
  • Licensed Clinical Social Worker (LCSW) with 4 to 6 years of experience in a senior role, including at least three years in behavioral health administration.
  • Strong leadership qualities are essential.

Schedule:

  • Monday to Friday, 8 AM - 5 PM

Salary Range:

$135,000.00 - $145,000.00
