Recreation Program Coordinator

The incumbent supports the Manager of Habilitation and Activities by implementing programs and initiatives that enhance wellness, recreation, and habilitation at Adult Day Health locations, in clients' residences, and through telehealth services. Under the guidance of the Manager, this role involves conducting thorough assessments and crafting personalized treatment plans tailored to each participant's physical, social, emotional, and spiritual requirements.
Key Responsibilities:
- Collaborates with the manager to design and develop programs and activities that promote wellness and habilitation. Coordinates the execution of daily activities alongside fellow Activities staff.
- Facilitates daily activities directly for participants at Adult Day Health, in their homes, and through telehealth platforms.
- Travels regularly between sites and conducts home visits for participants, ensuring transportation safety.
- Provides coverage at alternative Adult Day Health locations as necessary.
- Implements suitable interventions at Adult Day Health and within participants' homes.
- Creates and documents periodic participant assessments, progress notes, and care plans in line with organizational standards, ensuring timely completion.
- Maintains tracking spreadsheets and compiles information for inclusion in monthly calendars, newsletters, and various reports.
- Plans and executes special events such as holiday celebrations, therapeutic outings, entertainment, and other recreational activities.
- Prepares for and represents the Activities Department at interdisciplinary team meetings, communicating daily programming and any modifications to participants' care plans.
- Participates in pre-enrollment meetings, care planning sessions, debriefings, staff meetings, and discharge planning, attending family meetings as required.
- May provide coverage for Occupational Therapy services under the direction of an Occupational Therapist, adhering to relevant practices.
- Utilizes basic computer skills for Outlook, Word, and electronic medical records.
- Performs additional duties as assigned.
- Must be able to pass a fit test; the position requires the use of a mask where a proper seal is essential, and the incumbent must not have facial hair that interferes with the respirator seal.
Job Specifications:
- Completion of an Occupational Therapy Assistant curriculum accredited by the Accreditation Council for Occupational Therapy Education, Inc., with at least an Associate's Degree.
- Current Occupational Therapy Assistant licensure in the Commonwealth of Massachusetts.
- Current CPR Certification.
- A minimum of 1-2 years of experience working with the geriatric population.
- A minimum of 1-2 years of experience in program development.
- Ability to work collaboratively within a team and demonstrate consideration for colleagues.
- Covid vaccination is required.
