The incumbent plays a vital role in supporting the Manager of Habilitation and Activities by facilitating programs and events that enhance wellness, recreation, and habilitation at Adult Day Health locations, within participants' residences, and through telehealth services. Under the guidance of the Manager, this position involves conducting thorough assessments and crafting personalized treatment plans tailored to each participant's physical, social, emotional, and spiritual requirements.

Key Responsibilities:

  • Collaborates with the manager to design and implement programs and activities that promote wellness and habilitation. Coordinates the execution of daily activities alongside fellow Activities staff.
  • Delivers engaging daily activities directly to participants at Adult Day Health centers, in their homes, and via telehealth platforms.
  • Travels regularly between various sites and conducts home visits, ensuring transportation safety for participants.
  • Provides support at other Adult Day Health locations as necessary.
  • Implements suitable interventions at both Adult Day Health centers and participants' homes.
  • Develops and documents periodic participant assessments, progress notes, and care plans in accordance with organizational standards, ensuring timely completion.
  • Creates and maintains tracking spreadsheets, compiling information for inclusion in monthly calendars, newsletters, and reports.
  • Plans and executes special events, including holiday celebrations, therapeutic outings, entertainment, and other recreational activities.
  • Prepares for and represents the Activities Department at interdisciplinary team meetings, effectively communicating daily programming and any modifications to participants' care plans.
  • Participates in pre-enrollment meetings, care planning sessions, debriefings, staff meetings, and discharge planning, attending family meetings as required.
  • May assist with Occupational Therapy services under the direction of an Occupational Therapist, adhering to applicable practices.
  • Possesses basic computer skills for utilizing Outlook, Word, and electronic medical records.
  • Performs additional duties as assigned.
  • Must be able to pass a fit test, as the position requires the use of a mask where a tight seal is essential. The incumbent must not have facial hair that interferes with the respirator's seal.

Job Specifications:

  • Completion of an Occupational Therapy Assistant curriculum accredited by the Accreditation Council for Occupational Therapy Education, Inc., with a minimum of an Associate's Degree.
  • Current Occupational Therapy Assistant licensure in the Commonwealth of Massachusetts.
  • Current CPR Certification.
  • 1-2 years of experience working with a geriatric population.
  • 1-2 years of experience in program development.
  • Ability to work collaboratively within a team environment while being considerate of colleagues.
  • Covid vaccination is required.
