About the Role

We are seeking a compassionate and dedicated Care Manager to join our team at Sunrise Senior Living. As a Care Manager, you will be responsible for providing exceptional care and services to our residents in our assisted living and reminiscence neighborhoods.

Key Responsibilities
  • Build Meaningful Relationships: Develop strong relationships with a specified number of seniors and their families, providing assistance with activities of daily living and attending to individual care needs.
  • Individualized Service Plans: Participate in the development of Individualized Service Plans (ISPs) and monthly updates to ensure each resident receives personalized care.
  • Resident Care and Support: Provide physical checks on residents throughout the shift, ensuring their safety and well-being.
  • Collaboration and Communication: Partner with the community team to ensure compliance with OSHA requirements and promote Risk Management programs and policies.
  • Customer Service: Respond to the dining needs of residents and guests, maintaining Sunrise hospitality and service standards.
Requirements
  • Dedication to Senior Care: Passionate about serving seniors with excellent customer service skills.
  • Qualifications: High School diploma/GED accepted, with certifications and licenses required in states where applicable.
  • Experience: Previous experience working with seniors preferred.
  • Skills: Ability to make choices, decisions, and act in the resident's best interest, with strong written and verbal communication skills.
