Nursing Care Management Lead - RN
VillageCare is seeking a dedicated and driven RN with prior experience in Care Management for the Nursing Care Management Lead role. This position entails overseeing team members, enhancing productivity, managing the care transition initiative, and ensuring that care strategies effectively meet the health and psychosocial requirements of our members.
Key Responsibilities:
- Oversee and guide the activities of the care management team, ensuring effective coordination and communication among the Interdisciplinary Care Team members.
- Perform telephonic evaluations of members' conditions, assess and analyze re-evaluations, identify risks for recurrent hospitalizations, and monitor member progress towards established goals.
- Ensure comprehensive documentation of care plans, care management activities, and any complaints or appeals.
- Manage requests for authorization of covered services in accordance with regulatory standards.
- Assess discharge plans and the suitability of discharge statuses in collaboration with the Utilization Management department.
- Develop and implement workflows for the Chronic Care Improvement program and the Quality Improvement initiative.
Qualifications:
We invite candidates who possess a Bachelor's degree and hold an unrestricted NYS RN license. We are particularly interested in individuals with over a year of practical supervisory experience in care management within a managed care environment.
