Job Overview

POSITION OBJECTIVE

Under the supervision of the Assistant Director of Nursing, the Staff Nurse in Community Care delivers professional nursing services to patients in both residential and non-residential settings throughout New York City. This role involves diagnosing and addressing health issues through various nursing methodologies, including case identification, health education, counseling, and the provision of supportive and restorative care. The nurse will assess, plan, implement, and evaluate individualized care plans for patients.

KEY RESPONSIBILITIES

  1. Deliver specialized nursing services as outlined in the treatment plan approved by the physician, including initial evaluations for clients with various medical conditions.
  2. Recognize and differentiate between physical and psychosocial symptoms, selecting appropriate therapeutic interventions for effective nursing care.
  3. Continuously develop and assess nursing care plans for assigned patients, which may include behavioral health assessments, wound care, medication education, and family counseling.
  4. Maintain comprehensive patient records, ensuring compliance with required documentation standards, including OASIS.
  5. Document and update nursing care plans and progress notes to guarantee seamless patient care, making adjustments as necessary.
  6. Communicate patient responses to the healthcare team and facilitate referrals to other specialists when needed.
  7. Ensure continuity of care by collaborating with other healthcare professionals involved in the patient's treatment.
  8. Provide mentorship and guidance to nursing staff in delivering patient care.
  9. Engage in departmental meetings and interdisciplinary discussions regarding nursing practices and policies.
  10. Promote the agency's philosophy and objectives to patients, families, and community groups.
  11. Enhance professional skills through ongoing education and training opportunities.
  12. Participate in quality assurance and performance improvement initiatives as required.
  13. Perform additional nursing duties as assigned by nursing administration, including clinical record assessments.
  14. Deliver individualized nursing care to patients requiring intermittent professional services, using the nursing process effectively.
  15. Evaluate patient needs and develop comprehensive treatment plans, ensuring ongoing assessment of family requirements for additional services.
  16. Provide nursing services as per the treatment plan authorized by the physician, including emergency interventions when necessary.
  17. Document the physical, social, and environmental needs of patients, monitoring their condition and escalating concerns as appropriate.
  18. Incorporate input from patients, caregivers, and healthcare providers to create feasible Nursing Care Plans that align with available resources.
  19. Review and modify care plans in accordance with physician directives.
  20. Coordinate discharge planning activities and communicate with all relevant parties, including the patient's physician.
  21. Collaborate with case managers and other professionals regarding patient progress and discharge strategies.
  22. Exhibit professionalism and ethical conduct in line with departmental standards.
  23. Involve patients and caregivers in setting care goals.
  24. Foster effective working relationships with healthcare staff, physicians, and community organizations.
  25. Attend regular team meetings and participate in case reviews.
  26. Complete physician orders promptly and maintain accurate patient documentation.
  27. Ensure patient files are up-to-date, reflecting all services provided and care plans.
  28. Write clear and concise progress notes that document care implementation and patient responses.
  29. Demonstrate competence in patient education and medication administration.
  30. Adhere to safety and accuracy standards in treatment and procedures.
  31. Participate in interdisciplinary team efforts to develop and implement comprehensive care plans.
  32. Supervise and monitor the care provided by home health aides, ensuring proper documentation.
  33. Engage patients and families in educational activities related to care and assessment.
  34. Maintain thorough and accurate documentation in patient care records, utilizing technology as necessary.
  35. Perform wound care and other nursing procedures as required.
  36. Provide education on disease management and medication adherence.
  37. Utilize evidence-based practices in home care settings.

Minimum Qualifications

For individuals holding an unrestricted registered professional nursing license prior to a specified date, a valid New York State license and current registration as a Registered Professional Nurse is required, along with certification in Basic Life Support (BLS).

For individuals licensed after a specified date, a valid New York State license, current registration, and a Bachelor's degree in Nursing or higher from an accredited institution is necessary, along with BLS certification.
