A well-established and respected health plan, dedicated to fostering strong relationships with its members and providers, is seeking a skilled RN Team Leader to lead its Medicaid Care Management team.
About the Role:- Oversee Medicaid team operations, ensuring adherence to contractual requirements and quality outcomes.
- Provide clinical supervision and guidance to Care Managers and support staff.
- Manage care management workflows, including referrals, interventions, and documentation.
- Conduct quality reviews, track performance metrics, and implement improvement strategies.
- Coordinate interdisciplinary team meetings and collaborate with Behavioral Health.
- Resolve operational issues and ensure compliance with policies and procedures.
- Lead staff training and development efforts.
- Develop and implement care management strategies to improve patient outcomes.
- Collaborate with cross-functional teams to ensure seamless care coordination.
- Monitor and analyze performance metrics to identify areas for improvement.
- Provide coaching and mentoring to Care Managers to enhance their skills and knowledge.
- Stay up-to-date with industry trends and best practices in care management.
- Bachelor's Degree required; Master's preferred.
- 5+ years of Care Management experience in healthcare or Managed Care.
- 2+ years of leadership experience in Managed Care or healthcare.
- Proficiency in Microsoft Office and navigating healthcare systems.
- Current NY State RN license required.
- Competitive salary range: $105,000 to $115,000 per year.
