Role of a Care Coordinator

As a Care Coordinator at Village Care, you will be instrumental in various aspects of care management, including care planning, coordination, and health evaluations. You will monitor the quality and effectiveness of services, track progress towards individual health goals, and ensure optimal care for our patients. Your duties will also encompass overseeing patient transitions, facilitating seamless discharge planning from healthcare facilities.

By identifying health, environmental, and psychosocial challenges, you will recommend and implement strategies in collaboration with the Interdisciplinary Care team. This position provides a dynamic opportunity to significantly enhance the well-being of our diverse patient population while working in a customer-oriented environment.

Qualifications for Success

To thrive as a Care Coordinator at Village Care, candidates must hold a valid NYS RN License and possess a minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings. An Associate's degree or higher is required. Successful candidates will exhibit a passionate demeanor, outstanding communication skills, and strong problem-solving capabilities. Bilingual proficiency is essential, particularly in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole. Proficiency in navigating and utilizing various software and tools pertinent to care management is also a key requirement for this role.
