Position Summary:

The Clinical Transition Coordinator plays a crucial role in the healthcare team, facilitating the smooth transition of patients between the clinic and hospital settings. This position is responsible for managing admissions, discharges, follow-up care, and overseeing billing processes to ensure continuity of care.

Key Responsibilities:

  • Admission Coordination: Oversee the admission process for patients requiring hospitalization, ensuring all necessary documentation is completed efficiently.
  • Discharge Coordination: Manage the discharge process, ensuring clear communication of discharge instructions to patients and clinic staff.
  • Follow-Up Management: Schedule and coordinate follow-up appointments, ensuring patients are informed about their care plans.
  • Billing Oversight: Ensure accurate transfer of billing information between the hospital and clinic, addressing any discrepancies as needed.
  • Transitional Care Management: Facilitate post-discharge follow-up, including timely patient contact and coordination of subsequent visits.
  • Chronic Care Support: Assist in developing and implementing chronic care management strategies, including regular patient check-ins.
  • Communication Facilitation: Act as the primary liaison among the hospitalist team, patients, families, and clinic staff to ensure effective communication.
  • Quality Improvement Participation: Engage in initiatives aimed at enhancing the hospitalist program and improving patient care transitions.
  • Regulatory Compliance: Ensure adherence to all relevant healthcare regulations and organizational policies.

Qualifications:

  • High school diploma or equivalent required.
  • Certification as a Medical Assistant (CMA) is mandatory.
  • Basic Life Support (BLS) certification is preferred.
  • Additional certifications in chronic or transitional care management are advantageous.

Experience:

  • At least 2 years of experience as a Medical Assistant, ideally in a clinical or hospital environment.
  • Experience in patient admissions and discharge planning is highly valued.
  • Familiarity with medical billing and documentation processes is a plus.

  • Clinical Acumen: Strong understanding of clinical operations related to patient transitions.
  • Communication Skills: Excellent verbal and written communication abilities.
  • Organizational Skills: Strong multitasking capabilities with attention to detail.

  • Technical Proficiency: Familiarity with electronic health records (EHR) and medical billing software.
