Position Summary:
The Clinical Care Coordinator for the Hospitalist Program is essential in facilitating seamless transitions for patients between our clinic and hospital settings. This role encompasses managing chronic conditions and transitional care, coordinating admissions and discharges, and supporting the overall hospitalist program. The coordinator will engage in chronic care management (CCM) and transitional care management (TCM), while also performing various clinic responsibilities as required. Key focuses include accurate documentation of patient care, managing billing processes, and enhancing hospitalist operations to optimize patient outcomes.

Key Responsibilities:

  • Admission Coordination: Oversee the admission process for patients needing hospital services, ensuring all necessary documentation is completed efficiently.
  • Discharge Management: Facilitate the discharge process, clearly communicating instructions to patients and clinic staff, and ensuring resources are available for a smooth transition.
  • Follow-Up Coordination: Schedule and manage follow-up appointments for transitional and chronic care, ensuring patients are informed about their care plans.
  • Billing Oversight: Manage the transfer of billing information between the hospital and clinic, resolving discrepancies and maintaining accurate patient records.
  • Transitional Care Management: Contact patients post-discharge within the required timeframe, schedule follow-up visits, and coordinate care with healthcare providers.
  • Chronic Care Management: Assist in developing care plans for patients with chronic conditions, ensuring regular check-ins and effective coordination.
  • Support for Hospitalist Program: Act as a liaison between the hospitalist team, patients, families, and clinic staff to ensure optimal patient care.
  • Effective Communication: Serve as the primary contact for communication between the hospitalist team and clinic staff, ensuring clarity in patient needs and care plans.
  • Quality Improvement: Participate in initiatives aimed at enhancing care transitions and chronic care management, implementing best practices for improved patient care.
  • Regulatory Compliance: Ensure adherence to hospital and clinic policies, as well as healthcare regulations in all responsibilities.

Qualifications:

  • Active Registered Nurse (RN) license is required.
  • Basic Life Support (BLS) certification is mandatory.
  • Preferred additional certifications in chronic care management or transitional care management.

Experience:

  • At least 2 years of nursing experience, ideally in a hospitalist or clinical environment.
  • Experience with patient admissions, discharge planning, and transitional care is highly desirable.
  • Familiarity with medical documentation and billing processes is advantageous.

  • Team Collaboration: Ability to work effectively with the hospitalist team and other healthcare providers.
