Position Summary:
The Clinical Care Transition Coordinator plays a crucial role in the healthcare delivery process, ensuring smooth transitions for patients moving between the clinic and hospital settings. This position involves overseeing admissions, discharges, follow-up care, and the billing process, while also documenting patient interactions and facilitating both transitional and chronic care management.
Key Responsibilities:
- Admission Coordination: Oversee the admission process for patients requiring hospitalization, ensuring all necessary documentation is completed efficiently.
- Discharge Management: Facilitate patient discharges from the hospital back to the clinic, ensuring that discharge instructions are communicated effectively.
- Follow-Up Coordination: Organize follow-up appointments, including transitional and chronic care management visits, ensuring patients are informed about their care plans.
- Billing Oversight: Ensure accurate transfer of billing information between the hospital and clinic, addressing any discrepancies as needed.
- Transitional Care Management: Manage the transitional care process, including timely follow-up with patients post-discharge and coordination with healthcare providers.
- Chronic Care Management: Assist in developing and implementing chronic care management strategies, including regular patient check-ins and care coordination.
- Communication Facilitation: Act as the primary liaison among the hospitalist team, patients, families, and clinic staff to ensure clear communication.
- Quality Improvement Participation: Engage in initiatives aimed at enhancing the hospitalist program, identifying areas for improvement in patient transitions.
- Regulatory Compliance: Ensure adherence to all relevant healthcare regulations and clinic policies in all activities.
Qualifications:
- High school diploma or equivalent required.
- Certified Medical Assistant (CMA) certification is mandatory.
- Basic Life Support (BLS) certification is preferred.
- Additional certifications in chronic or transitional care management are advantageous.
Experience:
- At least 2 years of experience as a Medical Assistant, ideally in a clinical or hospital environment.
- Experience in patient admissions, discharge planning, and transitional care is highly desirable.
- Familiarity with medical billing and documentation processes is a plus.
Skills and Abilities:
- Clinical Insight: Strong knowledge of clinical operations, particularly regarding patient transitions.
- Effective Communication: Excellent verbal and written communication skills for interacting with diverse stakeholders.
- Organizational Skills: Exceptional organizational abilities with a keen attention to detail.
- Problem-Solving: Proficient in identifying and resolving issues related to patient transitions.
- Technical Proficiency: Skilled in using electronic health records (EHR) systems and medical billing software.
- Empathetic Approach: A compassionate demeanor focused on patient support during care transitions.
- Collaborative Team Player: Ability to work effectively within a multidisciplinary team.
