PRIMARY JOB RESPONSIBILITIES
  1. Conduct timely assessments of patients and their families, focusing on readmitted, high-risk, and resource-intensive patients. Triage between Case Manager and RN Case Manager based on the complexity of psychosocial, financial, and discharge needs.
  2. Evaluate patient and family adaptation to illness/disability and their ability to meet patient-centered needs. Assess the clinical course of patients to ensure ongoing care coordination. Confirm the necessity for acute hospital care and identify discharge obstacles.
  3. Collaborate with physicians, nurses, and other healthcare professionals to ensure a coordinated approach to patient care. Communicate medical plans, anticipated discharges, and alternative care settings with physicians. Address process issues to minimize delays in patient care and provide feedback to management for continuous improvement.
  4. Act as a liaison to external agencies such as home health, hospice, rehabilitation facilities, and public health services, addressing identified needs throughout hospitalization.
  5. Maintain clear and concise documentation in patient records, reflecting physical and functional limitations, psychosocial characteristics, educational needs, and discharge requirements. Initiate referrals as necessary, ensuring documentation aligns with post-hospital care needs and patient/family involvement.
  6. Prioritize patient goals and preferences, actively involving patients and caregivers in discharge planning. Ensure the discharge process aligns with treatment preferences and facilitates effective transitions to post-discharge care, reducing preventable readmissions.
  7. Assist patients and families in selecting post-discharge care providers by sharing relevant quality and resource utilization data aligned with patient goals.
  8. Demonstrate commitment to teamwork, fostering mutual goals and learning opportunities while promoting trust, respect, and positive morale.
  9. Seek consultation from peers and supervisors regarding challenging cases or deviations from care plans.
  10. Exhibit professionalism in conduct, adhering to Standards of Behavior as outlined by CCH.
  11. Maintain professional relationships with other departments, external organizations, service providers, physicians, and patient families.
  12. Ensure compliance with healthcare regulations and hospital policies, maintaining a clean record free from governmental sanctions.
  13. Fulfill other assigned duties as necessary.
  14. Conduct timely discharge planning evaluations to ensure appropriate arrangements for post-discharge care, avoiding unnecessary delays.
JOB REQUIREMENTS
  1. Education: Bachelor's degree in Social Work, Psychology, Human Services, or a related field.
  2. Licensure: None required
  3. Certifications: Adhere to Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
  4. Experience: Minimum of 2 years in the healthcare sector preferred.
  5. Technical Skills: Proficient hand/eye coordination for clinical tasks, clear verbal communication with patients, families, and staff regarding care.
  6. Interpersonal Skills: Strong verbal communication abilities; adept at mediating and interacting with diverse populations.
  7. Physical Requirements: Ability to lift/maneuver objects weighing up to 40 pounds in patient care. Shift work may be required.
  8. Mental Abilities: Capable of defining problems, collecting data, and drawing conclusions. Proficient in applying mathematical concepts to practical situations.
  9. Sensory Requirements: Adequate hearing and vision capabilities.
  10. Exposure to Hazards: OSHA CATEGORY III. Job duties do not anticipate contact with blood or contaminated materials.
  11. Work Hours: Up to 40 hours per week.
