Position Summary:
The Clinical Transition Coordinator plays a crucial role within our healthcare team, ensuring smooth patient transitions between our clinic and the hospital. This position is responsible for managing admissions, discharges, follow-up care, and overseeing billing processes between the two facilities. The coordinator will also document patient care, facilitate transitional and chronic care management, and support hospitalist operations to improve patient outcomes.
Supervisory Role:
- No direct supervisory responsibilities.
- Admission Coordination: Oversee the admission process for patients requiring hospitalization, ensuring all necessary documentation and pre-admission tasks are completed efficiently.
- Discharge Coordination: Manage the discharge process from the hospital back to the clinic, ensuring clear communication of discharge instructions to both patients and clinic staff.
- Follow-Up Coordination: Schedule and organize follow-up appointments, including transitional and chronic care management visits, ensuring patients are well-informed about their care plans.
- Billing Oversight: Ensure accurate transfer of billing information from the hospital to the clinic and assist in resolving any discrepancies. Maintain comprehensive patient records with up-to-date documentation.
- Transitional Care Management: Facilitate the transitional care management process, including timely follow-up with patients post-discharge and coordinating necessary follow-up visits.
- Chronic Care Management: Support the development and execution of chronic care management plans, including regular check-ins and coordination among various healthcare providers.
- Communication: Act as the primary liaison between the hospitalist team, patients, families, and clinic staff, ensuring effective and consistent communication.
- Quality Improvement: Engage in quality improvement initiatives related to the hospitalist program, identifying areas for enhancement in patient care transitions and implementing best practices.
- Regulatory Compliance: Ensure all activities comply with hospital and clinic policies, as well as relevant healthcare regulations.
- High school diploma or equivalent required.
- Certified Medical Assistant (CMA) certification required.
- Basic Life Support (BLS) certification preferred.
- Additional certifications in chronic care management or transitional care management are advantageous.
- Minimum of 2 years of experience as a Medical Assistant, preferably in a hospital or clinical environment.
- Experience with patient admissions, discharge planning, and transitional care is highly desirable.
- Prior experience in billing or medical documentation is a plus.
- Clinical Knowledge: Strong understanding of hospital and clinical operations, particularly in relation to admissions, discharges, and patient care coordination.
- Communication: Excellent verbal and written communication skills, capable of effectively interacting with patients, families, healthcare providers, and administrative staff.
- Organizational Skills: Strong organizational and multitasking abilities, with keen attention to detail and the capacity to manage multiple priorities.
- Problem-Solving: Ability to identify potential issues in patient transitions and develop effective solutions.
- Technical Proficiency: Familiarity with electronic health records (EHR) systems, medical billing software, and standard office applications.
- Empathy: A compassionate, patient-centered approach, with the ability to support patients through complex care transitions.
- Team Collaboration: Ability to work collaboratively within a multidisciplinary team, including physicians, nurses, and administrative staff.
