Patient Services Coordinator

About the Role:
The Patient Services Coordinator plays a vital role in ensuring seamless care coordination for our patients. As a key member of our team, you will be responsible for scheduling visits, communicating with field staff, patients, and physicians to maintain proper care continuity.
Key Responsibilities:
- Manage patient schedules, ensuring timely rescheduling and reassignment as needed.
- Initiate infection control forms and send completed reports to HRD for upload in the worker console.
- Provide backup support during busy periods, including receiving calls from field staff and assisting with weekly case conferences.
- Maintain client hospitalization logs, including entering coordination notes and sending electronic logs to relevant staff.
- Complete requested schedules as tasks appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction.
- Schedule discharge visits/OASIS collection or recert visits following case conferences when tasks appear on the action screen.
- Reschedule declined or missed visits (if appropriate) and process reassigned and rescheduled visits.
- Ensure supervisory visits are scheduled and run scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
- Prepare weekly agent schedules, performing initial review for productivity/geographic issues and forwarding to Branch Director for approval prior to distribution.
- Verify visit paper notes in the scheduling console as needed and assist with internal patient transfers between branch offices.
- Perform lab report assessment, fax copies to doctors, and route to Medical Records Department as necessary.
- May be required to perform patient visits and/or participate in on-call rotation (clinical role).
Requirements:
- Excellent communication skills and ability to interact well with diverse individuals.
- Strong organizational skills and ability to manage and prioritize multiple assignments.
- At least 1 year of home health experience.
- Prior packet review/QI experience preferred.
- Coding certification preferred.
