Overview
Engage in referral management activities within a healthcare environment and perform pre-admission assessments for patients.

Facilitate the referral process to guarantee a seamless transition of patients to post-acute care services within the Brigham and Women's Hospital (BWH) framework.

Gather patient evaluation data from diverse sources and relay information to relevant systems.
Establish and nurture collaborative partnerships with internal personnel and external referral entities.
Assess suitable discharge plans, communicate with referral contacts, and address customer inquiries with favorable resolutions.

Key Responsibilities

  • Conduct evaluations, interviews, and referrals for patients to the appropriate post-acute services within the BWH framework, adhering to established protocols and compliance with local, state, and federal regulations, including LTAC, IRF, and SNF.
  • Perform on-site clinical assessments to determine patient admission suitability.
  • Collect patient assessment data from various sources and ensure accurate transmission in designated systems.
  • Conduct patient evaluations through meetings and interviews with patients, families, or caregivers as necessary.
  • Foster collaborative relationships with internal teams to enhance customer satisfaction.
  • Maintain and develop referral source relationships to ensure ongoing customer satisfaction.
  • Support complex transfers utilizing BWH clinical and supervisory resources.
  • Confirm and qualify patient admissions as outlined in the liaison process.
  • Ensure admissions meet BWH criteria.
  • Communicate essential information from BWH to current medical providers to address patient and provider needs.
  • Identify and relay BWH referral influences to management for appropriate follow-up.
  • Evaluate discharge plan alternatives to determine the best approach for meeting patient needs.
  • Achieve targeted payor mix results as per established goals.
  • Meet account/territory referral and admissions objectives.
  • Share educational resources and updates with referral sources to promote understanding of BWH post-acute services.
  • Complete additional departmental, organizational, or network tasks in alignment with established objectives.

Qualifications and Experience
Education/Degree
Required
  • High School Diploma or equivalent.
Experience
Required
  • Minimum of 2 years experience in an acute hospital or extended care facility.
Licensure
Required
  • Current licensure in Massachusetts as RN, PT, OT, SLP (IRF requirement), LTAC, SNF may include LCSW; LICSW, MSW, BSW, LPN, PTA, COTA, RT.
Skills
Required:
  • Understanding of switchboard operations, paging protocols, and related equipment.
  • Proficiency in computer applications is essential.
Familiarity with Microsoft Office applications is preferred, along with the ability to learn new software.
  • Strong verbal, electronic, and written communication skills in English.
Preferred:
  • Comprehensive knowledge of various insurance plans and government reimbursement processes is advantageous.
  • Experience in sales and/or marketing is preferred.
  • Excellent communication and interpersonal skills, particularly when engaging with diverse populations.
  • Ability to work independently, demonstrate self-direction, and contribute effectively as part of a team.
  • Proactive in anticipating challenges and formulating strategies to address them.
  • High standard of service delivery, demonstrating initiative with the capacity to prioritize tasks, meet deadlines, and adapt to changing circumstances.
