Key Responsibilities
  • Facilitate discharge planning and length of stay oversight to ensure seamless patient transitions.

Requirements
  • Bachelor's Degree of Science in Nursing
  • Current License in the state of employment
  • 2-3 years acute care hospital setting experience, preferred
  • Care Management or Utilization Management experience, preferred
  • Certification in Case Management, Preferred
