Job Overview - Patient Resource Coordinator

The Patient Resource Coordinator undertakes essential administrative duties associated with patient care progression and transitions throughout the healthcare continuum. This role involves close collaboration with nurse care coordinators, social workers, physicians, and other members of the care team. The Patient Resource Coordinator serves as an advocate for patients and their families, aiming to uphold Brigham and Women's Hospital's commitment to delivering high-quality care, ensuring customer satisfaction, and optimizing resource utilization.

Key Responsibilities:

1. Provide comprehensive administrative support to the care team, patients, and their caregivers:

  • Assist with administrative tasks and communication related to post-discharge care, including follow-up appointments and coordination with long-term care facilities.
  • Handle administrative duties to facilitate the ordering of medical equipment and completion of necessary forms.
  • Ensure compliance with regulations by distributing key forms and documents.
  • Coordinate various types of patient transportation as directed by the care team.
  • Participate in family meetings and interdisciplinary discussions to contribute valuable insights related to responsibilities.
  • Document administrative tasks as guided by the care team.
  • Gather, confirm, and verify essential patient information, including demographics and healthcare preferences.

2. Maintain up-to-date knowledge of resources available to patients and their caregivers:

  • Act as a resource for post-acute care options, including insurance requirements and facility information.
  • Identify and refer patients to community services such as transportation, food assistance, and financial aid programs.
  • Engage in communication and collaboration with various social agencies, clinics, and community organizations as directed by the care team.

Qualifications:

A High School Diploma or GED is required. An Associate's or Bachelor's Degree is preferred, along with healthcare experience, particularly in extended care facilities and community organizations. Experience in hospital discharge planning, long-term care, or utilization review is advantageous.

Skills and Knowledge:

Strong interpersonal skills are essential for effective interaction with diverse staff levels, patients, families, and community organizations. Ability to function effectively within an interdisciplinary team is crucial. Candidates should possess extensive knowledge of regulations, community organizations, medical terminology, and healthcare systems. The role requires the capacity to manage a variable workload and adapt to changing priorities while working independently and proactively. Basic typing and computer data entry skills are necessary, along with familiarity with relevant software.

Flexibility in a dynamic environment is a must.
